A UK health and care visa from Nigeria costs far less than a Skilled Worker one. There is no Immigration Health Surcharge (IHS), and the fee is lower. Together that saves roughly ₦7,265,000 on a 3-year Certificate of Sponsorship (CoS), at the July 2026 rate. The catch: since 22 July 2025, care worker and senior care worker roles — the ones most agents advertise — are closed to fresh overseas applicants. Nurses and other CQC-regulated professionals can still apply directly from Nigeria. Here is who still qualifies, the real NMC-to-CoS sequence, and the ₦ cost.

How a UK health and care visa from Nigeria differs from Skilled Worker, in ₦

Three differences carry the weight.
- No IHS. A Skilled Worker applicant pays £1,035 a year, £3,105 for a 3-year CoS — about ₦6,197,000. A Health and Care applicant and their dependants pay none of it. That is most of the gap.
- Lower fee. £284 for a CoS of 3 years or less, against £819 on Skilled Worker — a further ₦1,068,000 saved. The employer also skips the Immigration Skills Charge (£364–£1,000 a year on Skilled Worker), which explains why more care and health employers can afford to sponsor at all.
- Narrower occupation list. Skilled Worker covers any RQF level 3-plus role, sponsored by any Home Office-licensed sponsor. Health and Care Worker is limited to nursing, medicine and set social-care roles. Only a CQC-regulated provider, an NHS trust, or the equivalent regulator in Scotland, Wales or Northern Ireland can sponsor it. Skilled Worker also carries a flat salary threshold £38700 when the rule came in, £41,700 from 2026 — a figure that never applied to this route. How to get a UK Health and Care Worker visa from Nigeria: NMC, CBT, OSCE, then a CQC employer
In short: a Nigerian nurse’s route runs NMC registration → CBT → OSCE → a job offer from a CQC-regulated employer → CoS → visa. NCLEX-RN is the US/Canada exam and has no bearing on a UK application. Source: nmc.org.uk, “Register as a nurse or midwife trained outside the UK.”
- Meet the NMC’s IELTS/English requirement (IELTS or OET) before you apply.
- Apply to the Nursing and Midwifery Council (NMC) for registration. Some qualifications need an Ecctis comparability statement inside that application — check your case on nmc.org.uk first.
- Sit the CBT (Computer-Based Test) at a Pearson VUE centre in Lagos or Abuja.
- Pass it, then book an OSCE (Objective Structured Clinical Examination) date. Test centres change and most sit outside Nigeria — budget for travel and check the current list before you book.
- Pass the OSCE and clear the NMC’s identity, health and character checks to get your PIN — full UK registration.
- Apply to CQC-regulated providers, NHS trusts or eligible health employers. Once hired, your employer assigns the CoS through its own Home Office account, at no cost to you.
- Apply for the visa using that CoS reference.
Sequencing is the real trap. Chasing an OSCE date before a job offer wastes months, and so does job-hunting before your NMC registration clears — employers sponsor registered nurses, not nurses mid-registration. And park the NCLEX-RN vocabulary: it is a separate exam for a separate country.

The CoS costs you nothing on either route — the employer pays it. Not in the table: the TB test at an IOM-approved clinic, and the VFS biometric fee, both due on a stay over 6 months.
Paying from Nigeria: a naira card won’t clear the visa fee, even with no IHS to pay — UKVI still bills in dollars. Use a dollar card, a domiciliary-funded prepaid card, or someone abroad — the full decision tree.
The 2025/2026 care-worker restrictions, stated plainly
In short: since 22 July 2025, care worker sponsorship from overseas is closed for SOC codes 6135 and 6136 — sponsors must recruit from workers already in the UK. Nurses and other health-route occupations are unaffected. Source: gov.uk, Statement of Changes to the Immigration Rules, July 2025.
Before 22 July 2025, care homes recruited care assistants and senior care workers straight from Nigeria. That door is shut. Sponsors can still fill those two roles, but only from someone who already holds UK immigration permission — a worker switching employer, or one whose sponsor lost its licence. If an agent is still advertising a fresh “care assistant CoS” for someone with no UK visa, that cannot legally happen under this rule.
A separate rule predates that closure. Since 11 March 2024, care workers and senior care workers cannot bring a spouse, partner or children as dependants on this route. Nurses, doctors and other health professionals are not covered by that bar — they can still bring dependants, with the usual evidence.
The trust point: a CoS is assigned by a licensed sponsor through its own Home Office account. It is never sold. With overseas care worker sponsorship closed, any “CoS with maintenance” offer for a care-assistant job, priced in ₦ upfront, is selling something the seller cannot legally issue. Verify any employer free on the gov.uk sponsor licence register — some job ads still call it the “sponsor list,” same free lookup.
